Account Set UP

We hope that we have been able to answer your questions and concerns about our services and how they work. If you have additional questions, please feel free to contact us and we will gladly answer them for you.

Once you are ready, we need to have the following items completed before we can activate your account.

BIG DOG DISPATCH REQUIRED PAPERWORK

  1. A completed Agreement For Dispatch Services form completed and signed.
  2. A completed and signed Limited Power of Attorney form on file. (This allows us to represent you and your company in setting up your company with brokerage firms.)
  3. A completed Company Profile sheet. Please be as detailed as possible.
  4. If you have multiple units, please complete the Multiple Truck/Trailer Assignments form.
If you are already set up with a Factoring Company we need that information. If you are not and would like information on factoring, let us know.

YOUR COMPANY DOCUMENTATION

  1. A copy of your Motor Carrier Authority form.
  2. A copy of your W9 form. 
  3. A copy of your Insurance Certificate. We require $100,000.00 in cargo and $1 Million in Liability Insurance coverage as a minimum. Please have your Insurance Agent list Big Dog Dispatch as a Certificate Holder:
